Accident Reporting

These guidelines are for reporting flight related accidents, for required record keeping, for education and for iniiating mitigation activities if possible. Serious accidents will also iniate USHPA accident reporting.
Qualifying Accidents
- an accident merits reporting if it meets any one of the following conditions:
- Moderate or severe injury (beyond scraped knees)
- Moderate or severe equipment damage (beyond bent contraol frame or bent battens)
- Caused by conditions that can be mitigated (obstacles, spectators, etc)
- Is a situation that offers useful lessons to be learned (explain WHY not just WHAT).
Reporting Procedure
- Reports falling into the first three categories above MUST be emailed to the club officers at hynerfreeflight@gmail.com. It is recommended that the last two categories also be posted to the hyner club forum (groups.google.com/g/hynerclub) for purposes of group discussion.
If falling in the first three categories please include the following information:
- Name
- date, approximate time of accident
- site
- weather conditions
- Contributing circumstances for mitigation (if applicable)
- Description of accident with analysis of why it happened
- Any damage or injury that occured
If your accident falls under the first two categories, we will ask that you (or an appointed designee such as the safety officer) also submit a report to USPA. (USPA Accident Reporting).
